  **Job Description:**
  
  - Be a vital part of the healthcare landscape in Solano County, California!
  - Provide compassionate and personalized care in the comfort of patients' homes.
  - Prioritize start of care visits to make a significant impact on patient recovery.
  - Supervise Physical Therapy Assistants, guiding them in delivering exceptional therapy.
  - Ensure timely reporting of start of care visits (next day mandatory).
  - Maintain detailed clinical notes and documentation within 48 hours of your visits.
  - Communicate effectively and routinely with your supervisor, intake, and office team.
  - Be responsive during normal business hours to foster teamwork and collaboration.
  
  **Requirements:**
  
  - Valid California PT License.
  - A minimum of one year of Home Health experience.
  - Willingness to expand service territory as needed.
  
  **Schedule:**
  
  - Full-time position offering 40 hours per week, Saturday through Wednesday.
  
  **Training:**
  
  - Enjoy three full days of training, one in-house and two in the field for hands-on learning.
  
  **Technology:**
  
  - Utilize Kinnser as your documentation system.
  - Equipment provided includes an iPad and Verizon One Talk for easy communication.
  
  **Compensation:**
  
  - Mileage reimbursement available at $0.655 per mile (from first to last patient).
  - Parking and toll fees reimbursed, ensuring no out-of-pocket expenses for you.
  - Option for a company car with a decreased bill rate.
  
  **Productivity Requirements:**
  
  - Achieve a productivity target of 30 points per week.
  - Understand that 1 point equals 1.33 hours (average workload of 8 hours per day).
  
  **Visit Types:**
  
  - Start of Care: 2 points
  - Recertification, Resumption of Care, OASIS Discharge, Evaluation: 1.5 points
  - Revisit, NON OASIS DC: 1 point
