Position is for an Associate Director, Resident Healthcare Programs to work at EAH Corporate in San Rafael, CA. Must have degree in public health, social work, healthcare administration, human services, public policy, or related field. 5 - 10 years progressive experience in affordable housing, healthcare programs, public health, social services, or integrated community-based service delivery. 4 years of supervisory experience. Must be a Certified Community Health Worker (CHW) or be able to complete the certification within first 90 days of hire. Salary range: $80,000.00 to $130,000.00 per year; hiring range for new employees is generally $80,000.00- $105,000.00 per year DOE.
Provides strategic leadership, oversight, and implementation of EAH's health and housing initiatives, including CalAIM, Community Health Worker (CHW) integration, Enhanced Care Management (ECM) coordination, Community Supports alignment, and emerging Medicare-related partnerships that support resident wellness and housing stability. Bridges affordable housing and healthcare systems by aligning Resident Services with Medi-Cal and Medicare-funded service models, managed care partnerships, and hub-based care coordination platforms. Ensures that EAH remains compliant, innovative, and scalable in delivering housing-based supportive services that address social determinants of health.
In collaboration with the Director of Special Programs, provides strategic leadership for the development, implementation, and oversight of EAH's CalAIM, Medi-Cal, Medicare-aligned, and health-focused initiatives across the portfolio, ensuring alignment with the Resident Services model and organizational priorities related to housing stability and resident wellness.
Designs scalable health-housing integration frameworks that address social determinants of health while maintaining compliance with housing regulations and healthcare partner requirements.
Identifies and advances innovative service models and funding opportunities that position EAH as a leader in integrated housing and healthcare systems.
Leads the creation and implementation of portfolio-wide Key Performance Indicators (KPIs) related to resident engagement, care coordination, health service access, housing stability outcomes, and partner performance.
Develops standardized workflows, documentation protocols, and service pathways for CalAIM, ECM, Community Supports, and/or Medicare-initiatives.
Works in close collaboration with the Director of Resident Services Policies, Systems and Operations to ensure alignment between health initiatives and agency-wide policies, procedures, and operational systems.
Supports the integration of data tracking tools, referral platforms, and reporting structures that enable consistent implementation and continuous quality improvement.
Serves as a primary liaison with Managed Care Plans, Medicare Advantage plans, healthcare providers, and community-based organizations to strengthen coordinated service delivery within EAH communities.
Leads the development and management of scopes of work, MOUs, and partnership agreements that define service expectations, compliance standards, and accountability measures.
Represents EAH in regional health-housing collaboratives and strategic initiatives that advance cross-sector partnerships and sustainable service models.
Leads the integration and oversight of Community Health Worker (CHW) and health-aligned roles within the Resident Services structure, ensuring role clarity, supervision frameworks, and compliance alignment.
Develops and delivers training and technical assistance to Resident Services staff on CalAIM, care coordination, trauma-informed engagement, documentation standards, and service boundaries between housing and healthcare systems.
Oversees compliance with CalAIM, Medi-Cal, Medicare-aligned initiatives, and related grant or funding requirements, ensuring accurate documentation, reporting, and program integrity.
Collaborates with leadership on budget planning, funding strategies, and expansion models to ensure continued growth and viability of integrated health-housing initiatives.
Maintains strategic engagement with high-need properties and pilot sites to ensure that system-level frameworks remain responsive to resident needs and operational realities.
Works closely with leadership in Resident Services and Real Estate Management to foster strong cross-departmental site teams.
Actively participates in EAH's Injury and Illness Prevention Plan.

Must have valid driver's license and DMV clearance.
Must have ready access to reliable vehicle and maintain own auto insurance in accordance with EAH minimum requirements.
Master's degree in Public Health (MPH), Social Work (MSW), Healthcare Administration (MHA), Public Administration (MPA), or related field
Experience in affordable housing environment.
Bilingual/multilingual, a plus.
