About GrooveMost people with osteoporosis, arthritis, or chronic pain get a diagnosis and a pamphlet. Maybe a referral to PT for six visits. Then they're on their own. Groove exists to close that gap.Groove is a remote, PT-led program that gives older adults with chronic musculoskeletal conditions an ongoing relationship with a clinician who knows them, meets them where they are, and helps them get stronger over time. We're early-stage, small, and building the kind of care most clinicians got into this field to provide.The RoleWe're hiring a California-licensed PT to own a caseload of patients and work with them over weeks and months, not visits. You'll build trust, design programs around their lives, and use new tools to deliver care in ways most PTs haven't experienced yet: AI, daily communication, remote monitoring, and telehealth. If you're curious about what's next for the profession, this is a chance to practice at the frontier of high-touch, tech-enabled care.You'll work alongside our Founding Clinician, collaborate with our physician team, and report directly to the CEO.What You'll OwnPatient Onboarding & CareConduct initial intake calls to understand each patient's goals, history, and limitationsBuild individualized, evidence-based programs focused on strength, balance, and bone healthProgress programs over time based on patient response and load toleranceOngoing Monitoring & EngagementCommunicate regularly with patients (primarily via SMS) to support, guide, and maintain momentumConduct periodic video check-ins to assess progress and deepen rapportAdapt programs in real time secondary to patient feedbackDrive adherence through consistent touch points and supportWho You ArePassionate about this population. We're a small team — good energy, clear communication, and genuine care for each other are the job.Background2-3 years of clinical experience, with:Populations: orthopedics, older adults, osteoporosis, or geriatric rehabSettings: outpatient, home health, telehealth, or digital healthRoles: independent caseload management; startup or remote care experience a plusCredentials: PT or DPT; active California license requiredWhat This Role Is (and Isn't)This is not a traditional clinic role. It's a flexible, remote role where you own patient outcomes and build meaningful longitudinal relationships. You'll have autonomy in how you work, with support from a small, high-caliber team.
