This role includes in-person services at our offices and home visits throughout San Francisco. The Case Manager partners with clients and their support networks to develop strengths-based care plans, coordinate services, advocate during crises, and monitor progress to ensure clients remain safely housed and connected to needed resources. This is a highly relational, community-based role for someone who brings clinical judgment, housing systems knowledge, and a deep commitment to LGBTQIA2S++ affirming and trauma-informed practice.Essential FunctionEssential Functions and Approximate Percentage of TimeApproximate Allocated TimeClient Assessment & Care Planning 50%Care Coordination & Advocacy 20%Documentation & Reporting 15%Team Collaboration & Professional Development 10%Client Transportation 5%RequirementsKEY RESPONSIBILITIES:Client Assessment & Care PlanningConduct intake, assessment, and enrollment for eligible clientsDevelop client-centered, strengths-based care plansCoordinate and authorize services based on assessed needsConduct quarterly home visits and monthly client contacts (minimum)Complete annual reassessments and discharge planningCare Coordination & AdvocacyCoordinate housing, medical, behavioral health, and social servicesAdvocate during crisis situations related to housing or healthCollaborate with community providers and public benefits systems (Medi-Cal, IHSS, Medicare, etc.)Partner with DAS centralized intake for referralsDocumentation & ReportingMaintain timely, accurate case notes and service documentationEnter and update required information in DAS OOA GetCare databaseSubmit monthly units of service reports to meet contract deliverablesTeam Collaboration & Professional DevelopmentParticipate in weekly case conferences and team meetingsAttend supervision meetings and external clinical consultationEngage in continuing education and trainingClient Transportation (Occasional)Provide transportation support to housing, medical, or benefits appointments as part of care coordinationEnsure client dignity, safety, and confidentiality during transportFollow all Openhouse vehicle use and safety policiesRequired Qualifications1–3 years of experience working with older adults and/or adults with disabilitiesOne year of demonstrated case management experience in community health, aging services, or homeless services systemsKnowledge of housing, medical, and psychosocial issues impacting LGBTQ+ older adultsStrong communication skills with clients, caregivers, and interdisciplinary teamsAbility to manage documentation and meet contract reporting requirementsAbility to travel locally throughout San Francisco for home visitsFluency in EnglishPreferred QualificationsBachelor's or Master's degree in social work, psychology, counseling, gerontology, or related fieldExperience in motivational interviewing, behavioral health, or advocacyFamiliarity with Medicare, Medi-Cal, IHSS, and behavioral health systemsExperience with government-funded case management systemsMultilingual proficiencyExperience working with older LGBTQIA++ adults or equivalent experience working with other populations with unique socialization and service needs including but not limited to dementia and Alzheimer's disease, behavioral health, caregiver wellness, traumatic brain injury, etc.Sensitivity to and knowledge of the unique life experiences, challenges, and strengths of the communities supported by Openhouse's mission.Expected Working ConditionsMust be able to travel locally to meet with clients offsite.Must be able to operate a computer and other common office machinery such as a copier/printer up to eight hours/day daily.Must be able to work in a shared office space, with moderate levels of noise.Ability to safely operate a motor vehicle and transport clients in a respectful, trauma-informed, and culturally responsive manner.Ability to assist clients entering and exiting a vehicle when appropriate (within safe practice limits).Ability to travel throughout San Francisco and surrounding areas as needed.Essential Functions & ComplianceThe responsibilities described above, including occasional transportation of clients, are considered essential functions of this position.
