Location: Walnut Creek, CA 94598

Schedule: Part-Time (Transitioning to Full-Time in July)

Hours: 3 days/week, 8:30 AM - 1:30 PM (flexible days)

Pay Rate: $30-$35/hour

Position Overview

We are seeking an energetic, personable, and customer-service-driven Patient Care Coordinator (PCC) to join a collaborative and growing medical team in Walnut Creek. This role will begin as part-time with a clear pathway to full-time hours in July as clinic volume increases.

The PCC plays a vital role in supporting both physicians and clinical staff, ensuring seamless coordination of patient care and daily operations. This is primarily an administrative position; however, flexibility is key, as the PCC may assist with back-office clinical responsibilities when needed.

Team Structure

Each physician is supported by:

  • 1 Medical Assistant (MA)

  • 1 Patient Care Coordinator (PCC)

This team-based model ensures high-quality patient care, efficient communication, and smooth clinic operations.

Key Responsibilities

  • Provide administrative support to physicians and Medical Assistants

  • Coordinate communication between patients, providers, hospitals, and external partners

  • Handle prior authorizations (including urgent/stat requests) daily - must be comfortable managing this independently

  • Monitor and respond to physician communications (messaging/text platforms)

  • Manage scheduling requests from referring providers

  • Conduct patient outreach for follow-ups and appointment coordination

  • Track and manage tasks using internal systems

  • Maintain detailed documentation for continuity of care

  • Draft and send professional communications (emails, messages, etc.)

  • Support clinic workflow and assist MAs with:

  • Rooming patients

  • Charting

  • Procedural support

  • Utilize EMR systems and clinic technologies efficiently

Qualifications

Required Skills & Attributes:

  • Strong patient-service mindset and professionalism

  • Excellent communication and interpersonal skills

  • Highly organized with the ability to multitask effectively

  • Comfortable working in a fast-paced, team-oriented environment

  • Confident communicating with physicians, patients, and vendors

  • Proactive, adaptable, and eager to learn

  • Ability to compose professional emails

  • Prior authorization experience is REQUIRED, with the ability to manage requests confidently and independently

Preferred Experience:

  • Medical office or healthcare front-office experience

  • Familiarity with EMR systems

Education/Certifications:

  • Not required; healthcare-related certifications are a plus

Compensation & Benefits

  • $30-$35/hour

  • 3 weeks PTO
