We are seeking a dedicated Physician (MD/DO) to provide comprehensive, coordinated primary care for participants in a PACE (Program of All-Inclusive Care for the Elderly) setting. Schedule: Monday Friday, 8:00am 5:30pm
Location: El Sobrante, CA

Key Responsibilities

• Perform comprehensive initial, 6-month, annual, and periodic assessments.
• Develop, review, and update individualized care plans in collaboration with the interdisciplinary team (IDT).
• Provide primary care, ongoing medical management, and coordinate diagnostic tests, medications, and specialty referrals.
• Participate in IDT meetings, care conferences, and family meetings.
• Provide after-hours telephonic on-call coverage as part of rotation.
• Coordinate care for hospitalized and nursing home participants; support smooth transitions and follow-up.
• Document all clinical interactions in the EMR (eClinicalWorks).
• Lead and participate in provider meetings, task forces, and quality improvement initiatives (QAPI).
• Maintain accurate medical records and ensure compliance with all policies, safety protocols, HIPAA requirements, and training standards.
