Role Description

This is an on-site, full-time position for a Medical Case Manager located in Walnut Creek, CA. The Medical Case Manager will oversee and coordinate the medical care and services for clients, manage disability and utilization processes, and deliver comprehensive case management services. The role includes assessing client needs, developing care plans, collaborating with healthcare providers, and ensuring all services align with the best interest of the clients while staying compliant with regulatory requirements.

Qualifications

  • Expertise in Medical Case Management and Case Management with a strong ability to assess and address client needs.
  • Experience in Disability Management and managing Case Management Services.
  • Familiarity with Utilization Management processes and adherence to regulatory compliance.
  • Strong interpersonal, organizational, and communication skills, with the ability to work effectively as part of a team.
  • Knowledge of medical terminology and healthcare systems.
  • Relevant certification in Case Management (e.g., CCM) is preferred.
  • Bachelor's degree in Healthcare, Nursing, Social Work, or a related field is preferred.

Manage a client caseload of 15-20 clients;

In field visits primarily in home or institutional settings;

Email and phone correspondence with clients, professionals, service providers, caregivers and/or family members;

Set up, evaluate, and monitor service needs on an ongoing basis;

Potentially manage all aspects of a client's life up to and including medical, housing, social, insurance, and financial needs;

Provide oversight and protection for vulnerable individuals;

This is a mandated reporter position.

Skills and Qualifications:

Must be able to pass basic background check;

Clean DMV and use of own insured vehicle (with mileage reimbursement);

Must be able to effectively utilize shared calendar;

Must be able to schedule own appointments and manage time effectively and efficiently;

Must be proficient in the use of office equipment and computer programs such as Microsoft Office, Outlook, and Client Management Platforms;

Experience in some type of service industry with special consideration given to service in the legal, health or social services areas;

Knowledge of social security, Medi-Cal, and Medicare benefits is a plus;

Bachelors degree in related field is a minimum.
