Role Description

This is an on-site, full-time position for a Medical Case Manager located in the San Francisco Bay Area. The Medical Case Manager will oversee and coordinate the medical care and services for clients, manage disability and utilization processes, and deliver comprehensive case management services. The role includes assessing client needs, developing care plans, collaborating with healthcare providers, and ensuring all services align with the best interest of the clients while staying compliant with regulatory requirements.

Qualifications

  • Expertise in Medical Case Management and Case Management with a strong ability to assess and address client needs.
  • Experience in Disability Management and managing Case Management Services.
  • Familiarity with Utilization Management processes and adherence to regulatory compliance.
  • Strong interpersonal, organizational, and communication skills, with the ability to work effectively as part of a team.
  • Knowledge of medical terminology and healthcare systems.
  • Relevant certification in Case Management (e.g., CCM) is preferred.
  • Bachelor's degree in Healthcare, Nursing, Social Work, or a related field is preferred.
