Independent Contractor | Star Nursing

Star Nursing is seeking reliable Care Managers to conduct monthly in-person visits with members residing in Residential Care Facilities for the Elderly (RCFEs) and Assisted Living settings within Contra Costa County.

These visits support care coordination by confirming member wellbeing, documenting observations, and communicating any concerns to the assigned Lead Care Manager for follow-up.

Contract Details

Pay: $25 per completed member visit

Schedule: Monthly visits for assigned members

Location: Contra Costa County facilities only

Assignments: Members and facilities provided by Star Nursing

Documentation System: Sprite care management system

Contract Type

1099 Independent Contractor

No mileage reimbursement or employment benefits are provided.

Contractors are responsible for their own transportation and travel expenses.

Reporting Structure

Reports To

Star Nursing Lead Care Manager or Program Supervisor

Collaborates With
• Star Nursing Care Management Team
• Residential Facility Staff

Care Managers provide in-person observation and documentation support and report findings to the assigned Lead Care Manager for care coordination follow-up.

Responsibilities

Conduct monthly in-person visits with assigned members residing in RCFEs or assisted living facilities.

Confirm member wellbeing and presence in the facility.

Observe and document any changes in:
• Physical condition
• Behavioral or cognitive status
• Living environment
• Member safety or support needs

Complete the monthly visit assessment in the Sprite system.

Report any concerns or changes to the assigned Lead Care Manager.

Maintain respectful and professional communication with facility staff and members.

Qualifications

Healthcare licensure is not required for this role.

Preferred experience includes:
• Work with seniors, individuals with disabilities, or vulnerable populations
• Experience in care coordination, case management support, or community programs
• Strong observation and documentation skills

Additional Requirements
• Reliable transportation to travel within Contra Costa County
• Ability to work independently
• Basic computer skills for electronic documentation

Pay: $25 per completed member visit

Role Clarification

Care Managers performing these visits do not provide clinical care or medical treatment.

Their role is to observe, document, and communicate concerns to the Lead Care Manager, who oversees member care coordination.
