Description:

Ensures over-all compliance with all state and federal laws and regulations in the provision of care and services rendered, in accordance with facility policies, procedures, and professional standards of care. Oversee Medication Management Program and supervision of Medication Aides. Observes residents for changes in their physical, mental, emotional and social functioning.

ESSENTIAL JOB FUNCTIONS:

  • Responsible for medication management program, ensuring compliance with state regulations. Ensure appropriate and accurate processing of all physician orders.
  • Monitor medication room to ensure proper receiving, ordering, storage, security and disposal of medications.
  • Supervision of Medication Aides.
  • Completion of Medication Training with ability to train Medication Aides.
  • Reviews 24 hour report on a daily basis and ensures appropriate documentation is complete
  • Attends change of shift daily.
  • Conduct weekly skin rounds to evaluate progress and effectiveness of current treatments. Recommendations made as indicated. Report to administrator weekly.
  • Complete any treatments which require a licensed nurse.
  • Assess residents for need of blood glucose monitoring, intake and output.
  • Complete assessment on all new residents within seven days of admission.
  • Reviews pre-placement appraisal, physicians report, any other accompanying information, converse with care staff and conduct interview with resident.
  • Completes fall, smoking and/or wandering assessment, as indicated
  • Complete resident assessments, identify care needs and initiate/update care and service plans as indicated.
  • Provide care plan to administrator for review, signature and completion of any needed negotiated risk agreements.
  • Conducts pre-admission appraisals as requested.
  • Assesses all residents every six months, or as required for changes of conditions or special needs.
  • Upon completion of annual assessments, gives list of conferences to be scheduled to receptionist. Coordinates and conducts care conference meetings.
  • Review, log, and report monthly weights and vital signs.
  • Verifies proper transcription of medication orders. Assist with medication distraction.
  • Makes rounds of entire building at a minimum of once per day, to identify any resident needs, deficits of care, etc.
  • Current knowledge of and compliance with all applicable federal, state and local laws, regulations, facility policies, procedures and licensing regulations, with on-going monitoring and review.
  • Conduct Department of Social Services Self-Assessment audits and make recommendations for continuous quality improvement.
  • Monitors the completion of required documentation of care and services, including assessments, physician orders, observations, interventions and response.
  • Utilizes 24-hour report form for continuing of care and attends change of shift report.
  • Completes accident/incident forms, and completes additional reports as required.
  • Promote and maintain good public relations.
  • Ability to interact effectively, provide leadership and act as a role model for residents and facility staff.
  • Maintain current knowledge of resident status.
  • Work with Administrator to ensure all care is provided according to physician orders.
  • Direct, practice, implement infection control program.
  • Ensure employees are following facility procedures for hand-washing, standard precautions and personal protective equipment to prevent the spread of infection and promote environmental health.
  • Ensure proper disposal of sharps.
  • Assist staff and problem-solving resident care issues.
  • Informs the resident, physician, administrator and family or responsible party when there was an incident, a significant change in physical, mental, functional or cycle social status; a need to alter care and treatment provided; a room or roommate change; and any other pertinent information.
  • Demonstrates knowledge of and respect for the rights, dignity and individuality of each resident and all interactions.
  • Maintains confidentiality of resident and facility information.
  • Promotes the highest possible quality of life and level of independence for every resident.
  • Acts as a resident advocate when necessary, and as a mandated reporter of abuse.
  • Reports relevant information to administrator including, compliance issues, quality improvement, resident risk factors, change of condition, medication errors, incidents, expressed complaint or dissatisfaction by resident or family, refusals of treatment, loss of property, unexplained injuries, and staff conduct.
  • Knowledge of emergency and disaster plan. Ability to remain calm and direct others in case of emergency.
  • Provide first aid, as necessary.
  • Performs other job duties as required by manager/supervisor.
  • High School Diploma or equivalent required.
  • Current, active CA LVN license
  • One-year experience in long-term care preferred.
  • Experience providing direct care to the elderly preferred.
  • Must pass medication dosage and calculation test with a score of 80% or higher
  • Must maintain current First Aid certification
  • Must maintain current CPR or BLS certification
  • Ability to receive and maintain a California Background Clearance through the Department of Social Services.
  • Good physical and mental health.

LANGUAGE:
  • Must be able to fluently speak, read and write English.
  • Fluent in Chinese (Cantonese and/or Mandarin) preferred
  • Fluency in other languages are an asset.
