Preference will be given to Chinese bi-lingual candidates. Successful candidate will have 4 ACPE units and experience in a Hospice environment. Competently assesses, coordinates and evaluates the spiritual needs of hospice patients and families and provides direct spiritual care in coordination with the patient care team. Develops, documents and delivers a hospice spiritual care plan. Fosters and maintains collaborative relationships between patients, their family/support group, physicians, and other healthcare providers through timely and effective communications and attendance at meetings and patient/family care conferences. Adheres to polices, industry standards, best practices, and applicable laws/hospice/ Conditions of Participation, regulations and codes to promote a quality, highly reliable patient experience.
