The Family Partner will team with Project Permanence Clinicians and Community Liaisons to support youth and their families in meeting their treatment goals. The Family Partner will provide insight, expertise and relevant resources to treatment team and families to support delivery of the High Fidelity Wraparound model of treatment for children, adolescents and families with mental and behavioral health challenges. The Family Partner will assist clients and caregivers in accessing resources in the community that will help the family stabilize the home environment and manage barriers to treatment. ESSENTIAL FUNCTIONS: Participate in Project Permanence team meetings and meetings with families in the home or community, such as at schools and in the juvenile justice system, as indicated by the family needs.
