Reports To: Social Services Program Manager

Summary: Provides administrativeand operational support for the Support at Home (SAH) Program, which helps low- to moderate-income older adults and adults with disabilities remain safely in their homes through subsidized home care services. The Program Assistant plays a key role in client communication, documentation, eligibility processing, data entry, and coordination with care providers and Case Manager Assistants.

Essential Functions:
• Assists with client intake process, including responding to inquiries, distributing applications, and verifying supporting documents.
• Schedules client assessments, appointments, and follow-up calls in coordination with the Case Manager Assistants.
• Maintains accurate and up-to-date client files, both digital and physical, in accordance with agency protocols.
• Inputs and updates data in the program database.
• Communicates with home care providers to confirm service start dates, schedules, and client preferences.
• Generates letters, reports, and other correspondence related to program participation.
• Provides general administrative support, including filing, scanning, photocopying, and data tracking.
• Assists with community outreach and educational presentations as needed.
• Monitors and reports service utilization, unmet needs, and client feedback to the Program Manager.
• Ensures confidentiality and upholds HIPAA compliance and agency privacy standards.
• Performs other duties as assigned to support the smooth operation of the program.
• Prepares reports as required by funding sources.
• Performs other duties as assigned.

Qualifications:
• Associate's Degree in psychology, counseling, rehabilitation, gerontology, sociology, or related field. Bachelor's degree preferred.
• Experience in providing health, social or aging services in the community.
• Excellent verbal and written communication and presentation skills.
• Must be self-motivated, organized and able to multitask under minimal supervision.
• Strong organizational and time management skills with attention to detail.
• Excellent interpersonal and communication skills, including the ability to work with diverse populations.
• Proficiency in Microsoft Office (Word, Excel, Outlook) and ability to learn client database systems.
• Ability to maintain confidentiality and work effectively in a fast-paced environment.
• Bilingual and bi-literate preferred. (English, Chinese, Spanish, Vietnamese, Tagalog, etc.).
• Commitment to the mission of supporting aging in place and client-centered services.
• Must be able to travel via public transportation or personal vehicle for the performance of job duties. If driving, must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage as specified in Self-Help's policy.
