Medical Social Worker (MSW) – HCBA Waiver Program

Direct Hire Opportunity

San Diego County | Field-Based | Mileage Reimbursement

Medical Social Worker supporting high-risk patients in the Home & Community Based Alternatives (HCBA) Waiver Program.

What You'll Do:

• Manage a 60–65 participant caseload
• Conduct in-home, telehealth, and telephonic assessments
• Develop & implement Plans of Treatment (POT)
• Coordinate discharge planning, referrals, and community resources
• Complete monthly monitoring & annual reassessments
• Collaborate with interdisciplinary care teams

What We're Looking For:

• MSW from an accredited school
• 2+ years in managed care, case management, utilization review, discharge planning, or transitional/acute care
• Preferred: Medical Case Manager experience or county programs (IHSS, SD Regional Center)
• Strong MS Office & documentation skills (EHR experience preferred)
• Valid CA driver's license, insured vehicle & ability to travel (mileage reimbursed)
• Home office setup required
