We are currently seeking a Medical Social Worker (MSW) to join our Daly City, CA team. This role is key to supporting patients and their families as they navigate complex medical and social needs within the home health setting.

Responsibilities:

• Assess the psychosocial needs of patients and families to develop individualized care plans.

• Provide counseling and emotional support to patients and caregivers.

• Assist in connecting patients and families to community resources, financial assistance, and legal aid.

• Collaborate with nurses, therapists, and physicians to ensure comprehensive care planning.

• Document all services provided in compliance with agency policies and regulatory requirements.

Requirements:

• Master's degree in Social Work (MSW) - Required

• 2+ years of MSW experience in a Home Health or similar healthcare setting - Required

• Strong interpersonal, organizational, and counseling skills

• Valid Driver's License and reliable transportation
