Position SummaryAt Dream Youth Clinic we believe in youth-led, adult-guided collaboration and are committed to "meeting youth where they are," and assisting them along their journey to wellness by imploring principles of Positive Youth Development, belonging, and youth agency. The Youth Health Advocate position works therapeutically with highly vulnerable youth populations to provide individualized client navigation and is able to take initiative, multi-task, communicate effectively, collaborate and advocate for the needs of youth clients.
The Health Advocate Manager is responsible for overseeing and supporting the health navigation services offered at Dream Youth Clinic. This position manages a small team of Health Advocates, ensures quality service delivery, and provides direct care to youth clients when needed. The Health Advocate Manager upholds a trauma-informed, reproductive justice framework and supports Dream's mission to serve Black girls, Black youth, youth of color, and gender-expansive youth.
Duties & Responsibilities- Supervise and support Health Advocates, providing standardized community health worker workflows, processes, coaching, training, and regular check-ins to ensure effective service delivery.
- Developing and implementing all Health Advocate Department processes including but not limited to health advocate protocols, policies, client care workflows, quality assurance, collaborative client management processes.
- Organize and oversee collaborative client management meetings and processes with Dream partner organizations.
- Oversee caseloads, documentation practices, and adherence to care protocols across the health advocacy team.
- Carry a small caseload of high‑acuity youth for barrier assessments, care planning, and follow‑up
- Provide crisis de‑escalation support as needed
- Identify new partnership opportunities to enhance referrals and resource networks
- Collaborate with clinical and program and medical staff to align health advocacy with broader youth wellness goals.
- Serve as a lead resource for addressing complex youth cases and supporting health advocates with developing comprehensive care plans.
- Represent Dream Youth Clinic and participate in case conferencing and multidisciplinary meetings with community partners and youth-serving agencies.
- Lead the organization of health advocacy team administrative structure by supporting the development of systems to track service data, evaluate impact, and contribute to reports.
- Ensure accurate, timely documentation in the electronic health record (EHR)
- Coordinate and lead team meetings and staff trainings related to youth health advocacy.
- Assist health advocate staff to develop, manage, and track budgets and spending.
- Assist health advocate staff in the development of reports monthly, quarterly and yearly to ensure effective compliance with established standards, contracted goals, and objectives.
- Make and recommend personnel decisions including but not limited to hiring, promotion, transfer and assignment of staff, and imposition of discipline.
- Provide timely and high-quality completion of all contract deliverables.
- Use independent judgment and discretion to make decisions affecting the department and staff.
- Complete all required documentation and reports in a timely fashion, as required by contract guidelines.
- Participate in staff meetings, supervision, agency meetings, and staff training.
- Coordinate Young Men's Group and co-facilitator with the health advocate coordinator.
- Other duties as assigned.
Competencies- Community Health Worker certification or equivalent preferred.
- Bachelor's degree in Social Work, Public Health, or related field + 2 years of supervisory or leadership experience in youth-serving settings OR Associate degree + 2+ years experience as Community Health Worker.
- Demonstrated experience supporting youth navigating homelessness, the juvenile justice system, parenting, CSEC, and/or foster care.
- Experience managing teams or mentoring peers strongly preferred.
- Skilled in trauma-informed, harm reduction, and positive youth development approaches.
- Excellent verbal and written communication skills.
- Experience working with LGBTQ+, gender-expansive, and BIPOC youth.
- Ability to work both independently and collaboratively in a team environment.
- Able to drive for work-related duties (mileage and parking reimbursed).
- Must be available to work on-site Monday through Friday, with occasional weekend responsibilities.
Physical Conditions:
- Regularly required to stand, walk, climb stairs, bend, stoop, and lift up to 25 lbs.
- Requires use of computers and office equipment.
- Must be able to visually observe and interpret non-verbal behaviors of clients and staff.
