Position: Clinical Case Manager II
Salary: $32.60 per hour (Non-Exempt), annualized at $67,800.20 with employer-covered health benefits

Reports to: Clinical Program Director II or III

This position is with Conard House, Inc. Outpatient Services within the Supportive Housing Program. Qualifications:

  • Bachelor's Degree and 4 years of experience working in a mental health setting required. Up to 2 years of graduate professional education in a clinical field: Social Work (MSW), Marriage and Family Therapy (MFT), Clinical Counseling (PCC), Psychology (Ph.D., Psy.D.) may be substituted for the experience requirement on a year-for-year basis. OR AA plus 6 years of mental health setting experience with 2-year experience accrued post AA.
  • Must demonstrate case management skills and a thorough knowledge of mental health interventions, terminology, and experience with outpatient services billing.
  • Effective assessment, advocacy, organizational, and communication skills with support service housing required.
  • Experience working with diverse populations and within a diverse team.
  • Experience with Medi-Cal Billing and Epic database strongly preferred.
  • Knowledge of Supported Self-Management Model, Strength-Based, and Recovery-Based services a plus.
Responsibilities:
  • Provide individual rehabilitation counseling, psychosocial case management, care coordination, and crisis intervention.
  • Assist residents in managing bills, purchases, and other financial matters.
  • Offer a wide range of resources to residents, addressing problems in a supportive, constructive, and non-judgmental manner.
  • Admit and orient new residents to the program.
  • Assess residents' needs for medical and psychiatric evaluations, making appropriate referrals.
  • Provide mental health services to residents and document services in Epic according to Conard House and
Medi-Cal guidelines.
  • Develop and implement case management and money management plans and contracts.
  • Coordinate Plans of Care with other services being provided to the resident.
  • Advocate for residents in securing entitlements, clothing, healthcare, mental health services, and other needs. Ensure resident applications and forms are completed accurately.
  • Lead groups and activities focused on stable living, promoting interaction among residents and engagement with social programs.
  • Meet monthly billing targets.
  • Create and maintain a supportive, independent living environment.
  • Foster community building within support service housing and assist residents in accessing community resources.
  • Respond to crisis situations in accordance with established procedures.
  • Support residents in developing daily living skills.
  • Plan and implement strategies for placement upon discharge from the program.
