The Disabilities Services program provides comprehensive support for neurodivergent individuals and their families. We offer information, advocacy, care management, counseling, a vibrant social club, and residential support through the Gary Shupin Independent Living Community.
POSITION SUMMARY:
We are seeking a compassionate and empathetic Care Manager to join our Disability Services team. Under the supervision of the Director of Disability Services, the Care Manager is responsible for coordinating individualized care and community support services that promote clients' independence and well-being. This position assesses client needs, develops and implements person-centered service plans, and monitors progress toward established goals. The Care Manager also serves as a liaison among clients, families, staff, and community partners to ensure high-quality, coordinated, and goal-driven services.
