We are currently seeking a compassionate, organized, and team-oriented individual to join our home health team as a Full-Time or Part-Time (Per Diem) Home Health Aide (HHA) in San Francisco, CA.

Position Overview

We are seeking an experienced, self-driven Full-Time or Part-Time (Per Diem) Home Health Aide (HHA) for Health Care to join our dedicated team and provide exceptional care throughout the region. As an HHA, you will evaluate patients physical health, develop individualized treatment plans, and work to improve their mobility, strength, and overall quality of life. You will collaborate closely with our Clinical Director and Clinical Staff, ensuring the highest standards of care while providing support both remotely and independently in the field.

Responsibilities

• Delivers personalized care and hygiene support to patients in the comfort of their own homes.
• Focuses on the promotion of health, prevention of disease and care of the ill at home.
• Maintain accurate clinical records and submit documentation according to company policy using a company?provided device.
• Comply with all company policies and procedures to ensure high standards of care.

Requirements

• Home Health Aide (HHA) License to practice in California Required.
• Experience in Home Health or Hospice care 1 Year Required.
• Current CA Drivers License.
• Able to travel within a 1525 mile radius on a regular basis.

Pay

• $25 $35 per hour (depending on experience)
