About the Role

We are looking for a full-time Care Navigator to help us provide a seamless and supportive experience for our patients. Your role will support clinical operations and patient communication in a high-touch, tech-enabled practice.

Responsibilities

• Warmly answer and triage phone calls and emails from patients.
• Coordinate appointments and follow-ups to ensure smooth scheduling.
• Manage communication with outside providers and facilities, including document sharing and referral coordination.
• Organize and upload electronic documents into our EMR system.
• Facilitate medical records requests and ensure timely delivery to and from referring providers.
• Follow up on lab and imaging orders and ensure results are received and reviewed.
• Maintain accurate and up-to-date records using Elation EMR, including chart preparation and health maintenance tracking.
• Support clinicians by preparing electronic charts before visits.
• Monitor and restock office inventory as needed to support team workflows.
• Actively participate in staff huddles and team meetings to improve patient care and workflow.
• Collaborate with clinicians, medical assistants, and leadership to support optimal care and patient satisfaction.

Qualifications

• Associates or Bachelors degree required.
• Exceptional attention to detail and follow-through.
• Strong communication skills with a warm, professional demeanor.
• Adaptable, self-directed, and comfortable in a fast-paced environment.
• Previous experience in primary care, concierge medicine, or integrative care settings is a plus.
• Solid understanding of HIPAA and patient privacy best practices.
