Overview

Join our San Jose, CA team. PACE Social Worker (MSW). Pay Range: $46.15 – $52.70/hr.

Responsibilities

  • Provide care management, counseling, and advocacy within a community-based PACE program serving frail and older adults.
  • Serve as a key member of the interdisciplinary team, applying independent social work skills to support participants' long-term care needs.
  • Conduct psychosocial assessments and develop care plans.
  • Coordinate resources and facilitate care transitions.
  • Offer supportive counseling and promote whole-person care with attention to cultural competence and collaboration.

Qualifications

  • Master's degree in Social Work from an accredited college or university.
  • Current BLS and First Aid certification.
  • Fluency in Chinese (Cantonese and/or Mandarin) required.
  • Valid California driver's license with clean driving history.
  • Preferred: Current CA LCSW, 2+ years of experience in a health-related setting, and 1+ year working with frail or older adults.
