We are hiring a part time, 30 hours a week Service Coordinator who provides outreach, case findings, assessments, and care planning to seniors, disabled adults, and families within the scope of the San Francisco Senior Center. Provides services that attend to the psychosocial, health, financial, recreational, nutritional, and legal needs of center participants. Works with the San Francisco Senior Center (SFSC) Supervisor to plan programs and activities.If you enjoy partnering with people and are excited about the opportunity to make a difference every day, then this might be for you!

What You Will Do:

• Establishes and maintains relationships with San Francisco Senior Center (SFSC) participants, their families, caregivers, and neighborhood groups through directed one-on-one meetings, informal contact during lunch service, classes and events, and telephone email follow-up.
• Identifies and documents persons in need of services and maintains awareness of health status through established relationships or referrals by Center staff or instructors with subsequent ongoing follow-up. Maintains confidential case files documenting the progress and status of participants.
• Serves as liaison or advocate with outside social and health agencies for the clients.
• Provides individual casework for frail or at-risk participants through in-person sessions, email, and phone contact to counsel and evaluate participant needs.
• Provides information and referral, crisis intervention and counseling to individuals and groups as needed. Collaborates with and provides ongoing guidance to ADRC staff to support their work with participants and ensure alignment with program goals, internal protocols, and Department of Aging and Adult Services (DAS) expectations.
• Maintains accurate and up-to-date records in MySeniorCenter.com and GetCare on all participants receiving casework services. Ensures completion of required documentation and statistics in a timely manner, including the DAS required database program.
• Fulfills obligations to meet Units of Service in the Department of Disability and Aging Services (DAS) Social Services contract. Tracks unduplicated participants served and the Units of Service in the DAS contract for social services.
• Identifies barriers to full and complete provision of service needed by participants and effectively problem-solves to resolve these issues.
• Provides participants with current information on services, benefit programs, and other resources. Supports participants during transitions into different levels of care when appropriate.
• Works with Senior Center Supervisor and Community Services Staff Development and Training Committee to identify and develop training opportunities for site staff.
• Works cooperatively with Senior Center Supervisor and other site staff.
• Works with their Supervisor at assigned location to support planned programs and activities luncheons, classes, presentations and arranges for translation of written materials if funded by DAS Community Services contract.
• Performs other work as assigned.

Who You Are:

• Knowledge of needs of low income disabled and aging persons and issues related to aging. Knowledge and access to community resources available to meet the needs of SFSC participants and recipients of meals programs. Demonstrated proficient interpersonal, verbal, and written English communication skills. Instructional and presentation skills to present information. Organizational and documentation skills for notes, case plans, work plans, quality assurance, measurement of outcomes, and budget compliance. Interpersonal skills to establish and maintain effective working relationships. Leadership skills to provide guidance and motivation to staff. Proficient with Microsoft Office suite of products (Word, Excel, Outlook).
• Bachelor's degree in Social Work, Sociology, Gerontology, Psychology or related field required. Master's degree in related field preferred. Minimum of three (3) years of recent experience in a social services capacity assisting seniors and/or disabled adults. Bilingual in Cantonese/Mandarin preferred.
