Who we are La Clnica de La Raza is a community based health center committed to providing culturally appropriate high quality and accessible health care to the diverse communities of the San Francisco Bay Area We have spent over 40 years advocating for and creating a health home for the many that have been denied access to care As health care activists we are dedicated to making sure individuals who do not have health coverage get the same level of quality healthcare as those who have it From our genesis as a community health center in 1971 we understood that we cannot heal our communities without also addressing the economic and social factors that affect health Many times this requires that we go outside and provide services in other settings to build the bridge that links individuals to our health center for preventive and primary care Over 90000 individuals come to La Clnica because it is a welcoming place that addresses the whole person coordinating and connecting them to a broad network of services to improve and maintain their health and well being While we are still known for our activism and spirit of social justice we are also proud to have grown into a sophisticated provider of primary health care services with 35 sites across Alameda Contra Costa and Solano counties About the Position Under general supervision will conduct nutrition screening assessment and counseling a diverse population on various nutrition related diseases such as hypertension high cholesterol obesity diabetes as well as implement nutrition care plans patients with diabetes pre diabetes gestational diabetes and other high risk pregnancies hypertension obesity and hyperlipidemia Assist in curriculum educational development and implementation of material development teaching classes and quality improvement work Major Areas of Responsibility include but are not limited to Conducts nutrition screening assessment and counseling and implements nutrition care plans for low and high risk patients with diabetes pre diabetes gestational diabetes and other high risk pregnancies hypertension obesity hyperlipidemiaProvides individual nutrition and health counseling for those with overweight diabetes hypertension and dyslipidemiaMaintains documentation by charting SOAP notes in the progress notesMay develop and participation group nutrition education programs and medical group visits ie diabetes child obesity dyslipidemia hypertensionCollects and reviews material for education such as pamphlets audio visual aids books and handouts May assist in the development and evaluation of culturally relevant nutrition education materialsAssists in data entry and retrieval and other quality improvement efforts for the departmentPromotes site based employee wellness effortsMay provide in service nutritionhealth education to staff and community partnersParticipates in meetings related to nutrition clinical health education and wellness as requiredassignedOther duties as assigned by supervisorMinimum Job Requirements Requires completion of Dietetic and Food Administration Program from an accredited university; RD or RD eligible plus one year experience working in a community health care setting Masters in NutritionDietetics preferredMust hold a current BLS Basic Life Support Certificate You will have 30 day grace period to obtain your certificate from the date of hireKnowledge of contemporary principles and practices of nutrition nutritional value of foods interpretation and implementation of special diets and meal planning and preparation that is culturally appropriateKnowledge and experience with Motivational Interviewing Stages of Change Division of ResponsibilitiesInterpretation of anthropometric biochemical clinical and dietary information and determination of appropriate dietary advice that is culturally appropriateKnowledge of aspects of storing andor preparing foods properly; interpretation and implementation of special diets; scopes functions and responsibilities of local health departmentsAbility to develop implement and evaluate nutrition education activities and coordinate them with other departmental programsAbility to work across cultures and demonstrate support of diversity equity and inclusionBilingual EnglishSpanish requiredSalary 3536 4640 per hour
