Requirements to apply


• Master's Degree in Social Work from an accredited program

• Valid California driver's license and reliable transportation

• First Aid/CPR certification

• At least 1 year of experience working with frail or elderly populations

• Proficiency in EPIC EMR and understanding of social work practice scope

Full-time, leadership-focused social work role providing oversight, coaching, and care coordination for San Diego PACE participants in a collaborative, interdisciplinary environment.


Job Overview


We're seeking a dedicated and experienced MSW Supervisor to oversee Social Services operations at our El Cajon PACE site. This role provides both leadership to a team of Social Workers and Case Managers and hands-on support through assessments, care planning, and case management. The ideal candidate is an empathetic leader with excellent organizational skills, strong clinical judgment, and a passion for improving outcomes for older adults. You'll ensure compliance, promote interdisciplinary collaboration, and contribute to the quality of care delivery at the site level.
