Overview
Under the general direction and supervision of the Senior Program Manager, the I&A Specialist will provide information and assistance, connecting adults and adults with disabilities and caregivers to programs and services, including public and privately funded options. The role includes outreach activities in booths and public speaking engagements for educational purposes on aging and disability services in the community. It requires collaboration with internal and external services to advocate for consumer needs and ensure services are delivered.
Essential Functions / Responsibilities
- Provide one-to-one assistance for seniors and adults with disabilities to resolve barriers.
- Provide information and referrals to clients utilizing a wide array of resources, including services such as adult day health care, caregiver assistance/support, community services, health and wellness, education, emergency preparedness, employment, financial assistance, food/nutrition assistance, housing and shelter, in-home care, legal safety, recreation, senior centers, LGBT programs/services and transportation.
- Provide up-to-date information and clarification regarding programs such as Medicare, Medicaid, SF CalFresh, and formal supportive and social services.
- Provide translation assistance (verbal and written) to clients who cannot speak English, using a language interpreter as necessary.
- Complete applications, review and respond to correspondence, fill out forms, and make or confirm appointments.
- Conduct client needs assessments and evaluate client needs; follow up to ensure needs are met and document progress notes for quality and effective service delivery.
- Maintain accurate and up-to-date access to resources.
- Assist in the planning and implementation of a variety of special events and recreational activities for clients.
Reporting and Documentation
- Complete accurate program and client data collection, data entry, and required programmatic reports on a timely and consistent basis.
- Maintain confidentiality of all client records; organize case files in hard copy and electronic formats; comply with HIPAA privacy and security rules to the extent applicable; implement HIPAA requirements.
- Administer evaluation data collection and participate in program and agency performance management processes.
- Create and submit monthly activity reports and monthly metrics in the CARBON system by the 15th of the following month.
- Enter and submit required data into SF GetCare monthly.
- Submit Community Services Block Grant time studies to HAS/DAAS in February, May, August and November.
- Develop and deliver ad hoc reports as requested by HAS.
- Develop and deliver annual summary report of SOGI data collected by July 10th each year.
Community Engagement
- Actively conduct outreach activities and recruit clients to ensure full targeted enrollment and ADRC service objectives are met.
- Design and disseminate outreach materials and informational presentations to raise public awareness of long-term service and support options.
- Conduct outreach to a wide array of community members and organizations.
- Develop partnerships and establish communication and rapport between community agencies.

- Provide exceptional customer service.
- Represent the organization in a prudent and professional manner.
Safety
- Maintain personal safety, use personal protective equipment (PPE) and follow the organization's policies.
- Report all safety issues, accidents or injuries to your manager as a priority.

Position Qualification Requirements
Education: Bachelor's Degree in Social Work, Human Services or related field; or AS in a related field, or at least 3 years of social service delivery with elderly and/or adults with disabilities.
Experience: Experience in interacting with seniors and disabled adults, case management, group facilitation and recreational activities; commitment to social action, social justice, civic engagement and leadership development.
Requirements
- Current Physical Health Screening (every 2 years thereafter)
- Fingerprinting Clearance (DOJ)
- Current Immunizations (TB, Influenza, MMR, Pertussis) kept current
- Current CPR & First Aid Certificate (every 2 years thereafter)
- Valid California Driver's License (with current auto insurance and a clean driving record)

Physical Demands
Typical duties require sitting ~80%, walking 10%, standing 10%; frequent use of hands for typing and handling; ability to lift up to 25 pounds occasionally; may involve bending, reaching, and working with children on the floor for extended periods; vision requirements include close, distance, color, peripheral, depth perception and focus.
