Assists the Bereavement Program Coordinator with ensuring bereavement services are provided to the families/caregivers of hospice patients in accordance with the interdisciplinary plan of care. Bereavement counseling consists of counseling given to the individual's family after the individual's death. Bereavement counseling is a required hospice service, provided for a period up to one year following the patient's death. Bereavement counseling will ensure that the bereavement services reflect the needs of the bereaved.
