Information & Assistance Specialist (Bilingual English/Chinese)

Under the general direction and supervision of the Senior Program Manager, the Information & Assistance (I&A) Specialist provides information and assistance to adults, including adults with disabilities and caregivers, linking them to programs and services (public and privately funded). The role includes outreach activities such as booths and public speaking to educate on aging and disability services in the community. Works collaboratively with internal and external services to advocate for consumer needs and ensure service delivery.

Responsibilities

  • Provide one-to-one assistance for seniors and adults with disabilities to help resolve barriers.
  • Offer information and referrals using a wide array of resources (e.g., adult day health care, caregiver support, community services, health and wellness, education, emergency preparedness, employment, financial assistance, food/nutrition, housing, in-home care, legal safety, recreation, senior centers, LGBT programs/services, transportation).
  • Provide up-to-date information and clarification regarding programs such as Medicare, Medicaid, SF CalFresh, and formal supportive and social services.
  • Provide translation assistance (verbal and written) to clients who do not speak English; utilize interpreters as necessary.
  • Complete applications, review and respond to correspondence, fill out forms, and schedule/confirm appointments.
  • Conduct client needs assessments and document progress notes to ensure quality service delivery.
  • Evaluate client needs, follow up to ensure needs are met, and maintain accurate resource access.
  • Assist in planning and implementing special events and recreational activities for clients.

Reporting & Documentation

  • Complete accurate program and client data collection, data entry, and required programmatic reports timely.
  • Maintain confidentiality of client records; comply with HIPAA privacy and security rules where applicable.
  • Administer evaluation data collection and participate in program and agency performance management.
  • Submit monthly activity reports and metrics in the CARBON system by the 15th of the following month.
  • Enter required data into SF GetCare monthly.
  • Submit Community Services Block Grant time studies as required and deliver ad hoc reports as requested.
  • Develop and deliver annual summaries of SOGI data by the stated deadline.

Community Engagement

  • Conduct outreach to recruit clients and meet ADRC Service Objectives.
  • Design and disseminate outreach materials and presentations to raise awareness of long-term service options.
  • Engage with a wide range of community members and organizations; develop partnerships with community agencies.

Customer Service

  • Provide exceptional customer service and represent the organization professionally at all times.

Safety

  • Maintain personal safety, use PPE, and follow organizational policies.
  • Report safety issues, accidents or injuries promptly to the manager.

Teamwork

  • Interact positively with team members and facilitate positive change; collaborate with supervisors as needed.

Position Qualifications

  • Education: Bachelor's Degree in Social Work, Human Services, or related field; or AS in a related field; or at least 3 years of social service delivery with elderly and/or adults with disabilities.
  • Experience: Experience interacting with seniors and disabled adults; experience in case management, group facilitation, and recreational activities; commitment to social action, social justice, civic engagement, and leadership development.
  • Requirements:
    • Current Physical Health Screening (every 2 years)
    • Fingerprinting Clearance (DOJ)
    • Current Immunizations (TB, Influenza, MMR, Pertussis)
    • Current CPR & First Aid Certificate (every 2 years)
    • Valid California Driver's License with current auto insurance and clean driving record
  • Languages: Chinese Cantonese/Chinese Mandarin/English

Software & Tools

  • Microsoft Office (Outlook, Word, Excel, PowerPoint, Publisher) and basic internet tools.
  • Ability to use general office equipment (fax, copier, printer, phone, computer).

Physical Demands

Typical physical requirements include sitting about 80% of the time, occasional lifting up to 25 pounds, and ability to reach, bend, and perform tasks as needed. May require interaction with children and ability to work in varied environments.
