Job Details

• Conduct Medicare annual health assessments in clinic and at patient homes
• See 712 patients a day
• Perform H&Ps and document in EMR systems
• Collaborate with patients PCPs to ensure continuity of care

Requirements

• Active NP license in California
• Bilingual required (Vietnamese, Cantonese, Spanish, or Mandarin)
• Comfortable with EMR systems and electronic documentation
• Professional, flexible, and passionate about patient care
• Strong team player with a compassionate approach
