Program Description
Under the Housing Support Program (HSP) and in partnership with Alameda County, Office of Homeless Care and Coordination (OHCC) Housing Services, Felton Institute's Union Street, is a licensed residential program that provides supportive services to emerging adults (ages 18-59) in the dependent system. The site operates 24 hours a day, 7 days a week, providing personal and therapeutic care, habitation, developmental, and supportive behavioral health and wellness services for up to 24 residents.
The program is designed to provide residents with the support and coping skills needed to adapt and function successfully in a small, home-like setting. The program provides a structured learning environment, creating a positive context for behavioral change, self-management, and expanded coping strategies for the management of behavioral health symptoms as alternatives to drug and alcohol use and to support ongoing housing and avoiding eviction. The residents will be supported by the clinical/service treatment team with clinical/case management and ongoing assessment, ongoing development of support systems in the community, daily activities that support interaction between clients and the community-at-large, and ongoing linkages that encourage socialization and utilization of general community resources (art workshops, community garden, etc.).
Job Description
Under the supervision of the Program Manager, the Peer Support Resident Specialists are members of the multidisciplinary treatment team, with the role of providing coordinated care services for clients who reside at the Union Street residential program. Essential functions of the job include providing outreach and engagement, case management, ongoing assessment, and support with daily activities. Duties include assessing the needs, assisting in developing a care plan, authorizing services, coordinating service providers, daily client ID notes, following up and monitoring, reassessment, and discharge and termination. The staff will support the intake processes and screening, enrollment, comprehensive assessment, and care planning, using electronic charting systems. The staff also participates in collaborative staff meetings and meets regularly with the Program Manager and the team for case review and consultation, as well as participating in relevant training and collaborative efforts within the agency. Additional responsibilities include advocating with outside agencies on behalf of clients to secure benefits, housing, and resources, and maintaining a professional working relationship with providers and other support systems.
