Case Manager (Phoenix Rising Youth Home)

Ruby's Place dba Restorative Pathways (RP) is an innovative nonprofit committed to ending domestic violence, human trafficking, and violent crime through hope, advocacy and connection. Our services include emergency shelter, transitional housing, rental subsidies, mental health services, and wrap around services. All our services are trauma-sensitive and client-centered and are provided at no cost to our clients. There are no barriers to participate in our programs.

What qualifications are required for this position?
You Are Also:
- In possession of a Bachelor's degree in Social Work, Counseling, Youth Development, or related field (or equivalent experience).
- At least 21 years of age or older.
- 2-3 years of experience working with youth, particularly those impacted by trauma, interpersonal violence, human trafficking, or other challenges.
- Strong understanding of trauma-informed care and culturally responsive practices in youth engagement.
- Experience in mentoring, coaching, or supporting youth in educational or personal development contexts.
- Excellent communication, active listening, and motivational skills.
- Ability to work both independently and collaboratively as part of a multidisciplinary team.
- Experience working with youth in underserved communities or those affected by violence and trauma.
- Familiarity with community resources, job readiness programs, and educational pathways for youth.
- Bilingual or multilingual abilities are a plus.
- In possession of a valid CA driver's license, proof of insurance, and be approved by RP insurance as a driver in order to drive agency vehicles. If a staff member cannot be approved by RP insurance as a driver, employment might not be able to continue. Instances where staff cannot be approved to drive will be reviewed on a case by case basis to determine if employment can continue in their role without driving agency vehicles. Items considered include but are not limited to: the shift the staff member holds and the frequency/necessity of driving during that shift, the amount of time before the staff member may be approved to drive by the insurance carrier, the availability of other staff on site to drive agency vehicles as program needs require, etc.
- Able to complete all required background checks. Additional requirements may be deemed necessary, including fingerprinting (Live Scan), completion of all Community Care Licensing (CCL) documents, and ability to meet CCL licensing requirements.
What You Will Do
The anticipated start date for this position is fall 2025. The Case Manager (Phoenix Rising Youth Home) plays a key role in connecting with and supporting at-risk youth and/or young people who have experienced Human Trafficking and other forms of exploitation. This position is focused on building trust, sharing resources, and cultivating safety plans, educating youth and providers about human trafficking & exploitation. The Case Manager (Phoenix Rising Youth Home) will work closely with the youth to foster personal growth, build resilience, and work to connect them with supportive services to overcome barriers and the impact of human trafficking and exploitation.
Responsibilities include
- Provide emotional support, general case management, and advocacy for victims and survivors of human trafficking and exploitation
- Provide case management, including the development of action plans for residents, advocacy, information and referrals to community service providers.
- Conduct thorough needs assessments for youth and families (if applicable), focusing on safety, well-being, and access to basic needs.
- Advocate on behalf of youth in various systems (e.g., education, healthcare, legal).
- Conduct individual and group counseling and other activities for the youth at the facility.
- Orient new residents to the facility.
- Assist with general program operations.
- Conduct proactive outreach efforts in schools, community centers, and other spaces frequented by at-risk youth and their families.
- Build trusting relationships with youth and families to establish connections and assess service needs.
- Collaborate with community organizations, schools, and law enforcement to identify and support at-risk youth.
- Link youth and families to services, including counseling, housing, healthcare, education, legal assistance, and employment programs.
- Develop and maintain relationships with community partners to expand access to resources for youth and their families.
- Provide warm handoffs to service providers and ensure continuous care coordination.
- Liaise with county Social Services Administration regarding cases.
- Have knowledge of MediCal, CalFresh, CalWorks programs and eligibility requirements.
- Transport clients to and from legal, social services and other case management related appointments.
- Deliver services with sensitivity to cultural, racial, gender, and linguistic differences.
- Provide confidential, holistic, client-centered, trauma-informed services.
- Work with community partners around housing and legal resources for eligible clients.
- Engage in advocacy efforts to promote awareness of youth exploitation and at-risk populations.
- Represent the agency at community events, coalitions, and task forces focused on combatting youth exploitation.
- Document performance targets and outcomes for reporting purposes.
- Maintain accurate client records and notes
- Fill out a functional timesheet by assigned due dates and in compliance with the time allocations.
- Actively participate in reflective supervision.
- Create and maintain documentation of processes and procedures in relation to duties of the job.
- Other duties as assigned by your supervisor.
- Complete functional timesheets by assigned due dates in compliance with time allocations.
Working Conditions
This position is in person. You are expected to be available on-site as necessary given business needs. You must be able to stand for long periods of time and perform physical activities, including lifting up to 25 pounds. You will be subject to frequent interruptions and must be able to multitask and handle stress.
Local travel in the greater Bay Area may be required.

The pay scale for the Case Manager (Phoenix Rising Youth Home) is $30 per hour (non-exempt).
