Immediate need for a talented Care Management Support Coordinator II . This is a 04+ Months Contract opportunity with long-term potential and is located in CA (Remote). Please review the job description below and contact me ASAP if you are interested.

Pay Range: $21 - $22/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:
- CM Support Coordinators are assigned at least 30 members daily to outreach, CM Support Coordinator will do 1 call attempt per day to #s listed for member.
- If a # is invalid PC will also make a call attempt to PCP to attempt to get a valid #.
- If member is reached, CM Support Coordinator will review CareGaps with member, offer to assist with scheduling appointment, complete HRA if needed, and confirm member has no other questions/needs.
- 25 new members per day, 125 per week, and 500 per month.
- Audit score of 90% or better.
- Complete all outreach attempts within the 10 day maximum timeframe.
- Use proper note documentation.
- 80-100 Calls per day.
- Skills-Customer service 2 yr health care experience
- At least 1.75yr Experience with HIPAA procedures
- High volume inbound/outbound call experience
- Health plan experience, Customer service oriented.
- Team oriented, with room for advancement.
- High School Diplom
