The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco. The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services. We advocate for health equity and deliver innovative, high-quality services responsive to the neighborhoods and diverse communities we serve.
We are currently looking for qualified candidates for our Behavioral Health Case Manager role This position reports to the Director of Behavioral Health and Substance Use Counseling and is a regular, full Time, Non-Exempt position with a starting hourlyrate range of $29.36 to $31.46 with full benefits.
Primary Objective:
Under the supervision of the Director of Behavioral Health and Substance Use Counseling, the Case Manager performs screenings and short-term case management tasks for patients in the Prenatal Clinic, Adult Medicine and Pediatrics clinics. The Case Manager will endeavor to reduce the barriers patients encounter in meeting their needs around Social Determinants of Health.

Position Qualifications Requirements
Education
Bachelor's Degree in Social Work or related field, Or an Associates of Arts degree with 2 years of community or healthcare experience in care management activities.
