HealthRIGHT 360's Mobile and Street Medicine program delivers comprehensive, high-quality medical care and social services to individuals experiencing homelessness in San Francisco. Rooted in the mission to "go to the people", the program offers services directly to patients-on the street, in encampments, or via a fully equipped mobile medical van. The program operates as an extension of our Integrated Care Center (ICC), expanding the reach of our services beyond the clinic walls. This new and rapidly growing initiative is committed to providing low-barrier, trauma-informed, and person-centered care in the environments where people live. Our multidisciplinary team prioritizes trust, continuity, and access while delivering compassionate, dignified, and high-quality services to some of the city's most underserved populations.

This role will provide ongoing support and monitoring progress towards care plan goals for a specific caseload of clients served by this team. This role will take part in Enhanced Care Management (ECM) through SF health plans and the Integrated Care Clinic (ICC), which is HealthRIGHT 360's brick-and-mortar primary care clinic at 1563 Mission Street, San Francisco.
