Salary Range: $25-$27 an Hour

The Case Manager for Minors and Non-Minor Dependents in the Transitional Housing Program (THP) provides direct support and intensive case management services to assist Transitional Aged Youth (TAY) ages 16 to 21 years, helping them meet individualized goals and connect with community resources. The typical caseload is 12 youth. This position involves collaboration and care coordination within an interdisciplinary team.

All UC staff are responsible for adhering to performance standards set forth in the UC Staff Employee policies and procedures.

QUALIFICATIONS:

Required:

  1. Bachelor's Degree in areas such as Marriage, Family and Child Counseling, Child Psychology, Child Development, Counseling Psychology, Social Psychology, Clinical Psychology, Educational Psychology, Education with an emphasis on Counseling, Social Work, or Social Welfare, including counseling or psychotherapy content required for licensure as a Licensed Professional Clinical Counselor
  2. OR a Bachelor's Degree in Human Services or Behavioral Science with two (2) years of experience in a child welfare social services setting working with TAY aged 16-24
  3. OR CCL approval as an Excepted Case Manager prior to start date

Additional qualifications include experience working with the TAY population, knowledge of Microsoft Office applications, skills in case work, group work, and case coordination, understanding of transformational care planning and targeted case management, excellent interpersonal skills, and a positive outlook in treatment and care.

Preferred:

  1. Master's Degree in relevant fields, with two (2) years' experience working with at-risk youth or in a residential setting
  2. Bi-lingual in Spanish and English

RESPONSIBILITIES INCLUDE (but are not limited to):

  1. Program Implementation and Direct Client Services:
  • Establish and maintain relationships with youth using various approaches.
  • Maintain weekly contact, discuss goals, track progress, connect with community resources, and support development of independent living skills.
  • Participate in team meetings and ensure services align with the THP Program Statement.
  • Attend training, cover shifts as needed, and adhere to compliance and reporting standards.
  1. Compliance and Reporting:
  • Complete reports timely, input data accurately, and ensure documentation is up-to-date.
  • Maintain knowledge of CCL Regulations and ensure compliance, conducting regular audits.
  1. Program Productivity:
  • Meet productivity standards, maintain accurate case notes, and evaluate clients with proper documentation.
  1. Program Ambassador:
  • Serve as community liaison, interface with county staff, and develop community resources.
  1. Organization Duties:
  • Submit timecards timely, obtain pre-approval for time off, and stay informed about policies and procedures.

Other duties as assigned.

ESSENTIAL DUTIES AND QUALIFICATIONS:

Must be able to interpret documents, communicate effectively, apply instructions, resolve problems, and perform physical tasks including lifting up to 25 pounds. Certifications in CPR and First Aid are required within 10 days of hire, along with background checks, health screening, and a valid driver's license.
