Purpose Statement

This Counselor I position is for IOP/PHP, providing treatment and support to assist patients in their recovery from substance use, eating disorders, or other behavioral problems.

Responsibilities

  1. Conduct group and/or family sessions, under supervision, and document as required.
  2. Co-facilitate assigned sessions and assist with modifying problem behaviors.
  3. Perform initial and follow-up assessments for patients.
  4. Prepare individual treatment plans for patients.
  5. Complete all documentation regarding patient care, treatment, and incidents timely and clearly.
  6. Act as a liaison between referral sources and patients.
  7. Provide crisis intervention as needed.
  8. Plan for aftercare for patients.
  9. Coordinate with insurance carriers as requested.
  10. Provide case management to ensure individualized, quality care.
  11. Evaluate patient needs and determine if referrals are necessary.
  12. Maintain a positive, empathetic, and professional attitude towards patients, prioritizing patient safety.

Qualifications

  • High School diploma or equivalent required; Bachelor's in social or health services preferred.
  • Experience in counseling or related internship/practicum required.

Licenses/Certifications

  • State licensure or certification for counseling, or eligibility within designated timeframe.
  • CPR and de-escalation/restraint certification required (training provided).
  • First aid may be required depending on state or facility.

Additional Requirements

  • Ongoing education and training as required by state and accrediting bodies.
