Title:Activity Coordinator

Department: Adult Day Services

FLSA Status: Non-Exempt

Reports To: Director

Summary: Responsible for the activity program of the Adult Day Services Center; develops and implements individualized activity plans and provides Person-Centered Care activities for participants.

Essential Functions:
• Conducts initial, quarterly and 6-month assessments.
• Develops, implements, oversees, and evaluates the recreation and activity programs for the Adult Day Health Care (ADHC), Adult Day Care (ADC), and Alzheimer's Day Care Resource Center (ADCRC) Programs, with participants' involvement in the planning of the programs; posts the schedule of planned activities in a central location readily accessible to participants and caregivers.
• Leads and plans activities for participants.
• Maintains appropriate records according to the program's policies and procedures.
• Maintains the inventory of activity supplies and replenishes them as needed.
• Participates in morning huddles, multidisciplinary team meetings, staff meetings and other meetings as required.
• Performs other duties as assigned.

Qualifications:
• Bachelor's degree in the human services field and two years of experience in a social, recreational or educational program within the past five years (one year must be full-time employment in an activities program in a health care, mental health or handicapped program setting) OR;
• Be an occupational therapist, art therapist, music therapist, dance therapist, recreation therapist, occupational therapy assistant or qualified social worker OR;
• Completed at least 36 hours of training in a course designed specifically for this position and approved by the California Department of Aging and shall receive regular consultation from an occupational therapist, qualified social worker or recreation therapist.
• Knowledge of the physical, mental, and social needs of the elderly and disabled adults, and demonstrated aptitude for working with frail older adults.
• Experience in working in a multidisciplinary team and supervisory experience.
• Good organizational, interpersonal, and verbal and written communication skills.
• Bilingual and bi-literate in English and Cantonese.
• Proficient in MS Office and the Internet.
• Must have TB clearance within 12 months prior to employment and maintain clearance annually thereafter.
• Must acquire CPR and First Aid certificates within 6 months of employment and maintain current certificates thereafter.
• Must be able to travel via public transportation or personal vehicle for the performance of job duties. If driving, must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage as specified in Self-Help's policy.
