Under the supervision of the HHPS Associate Director, the Homelessness Prevention Specialist is responsible for serving households at the highest risk of homelessness or those experiencing homelessness. The role includes ensuring households receiving Housing Focused Case Management successfully connect to community resources such as legal services, mediation, public benefits, and behavioral health services from program enrollment to exit. Additionally, the specialist will handle administrative, financial, and record-keeping functions.

Primary Functions
ESSENTIAL FUNCTIONS / RESPONSIBILITIES
- Conduct initial problem-solving conversations with households seeking services.
- Identify creative solutions to prevent or resolve homelessness quickly.
- Develop and implement Housing Stability Plans in collaboration with households.
- Follow up and refer non-eligible households to appropriate resources or assistance programs.
- Refer households needing mediation or legal services to relevant agencies.
- Provide Housing Focused Case Management and connect households with other programs and organizations.
- Offer translation and interpreter services as needed.
- Complete Housing Stability Plans, including household budgets, for all households receiving services.
- Ensure households remain stably housed from enrollment to exit.
- Assist households in increasing their monthly income during the program.
- Provide budgeting and money management support or connect to related services.
- Facilitate external referrals and serve as a liaison to support services.
- Input data into reporting systems and prepare program reports.
- Maintain confidentiality of eligibility documentation in digital and hard copy formats.
- Conduct case monitoring to ensure housing stability and income growth.
- Gather feedback through annual surveys to evaluate service effectiveness.
- Prepare monthly, quarterly, and annual reports on program activities.
- Organize and facilitate relevant workshops.
- Enter required metrics into databases.
- Perform other duties as assigned.

- Provide exceptional customer service and represent the organization professionally.
Safety
- Maintain personal safety and follow safety policies, including PPE use.
- Report safety issues, accidents, or injuries promptly.
- Follow safety and emergency procedures to promote participant health and safety.
Teamwork and Collaboration
- Interact positively with team members and facilitate positive change.
- Partner with supervisors to address department issues.
Position Qualifications
Education: Bachelor's Degree in Education, Human Services, Social Work, Psychology, or related field, or equivalent work experience.
Experience: Minimum 6 months working with low-income or impacted populations, including those experiencing homelessness, domestic violence, or COVID recovery. SF ERAP experience is a plus. Ability to serve diverse populations and work independently.
Schedule: 40 hours/week, Monday to Friday, with periodic evenings and weekends.
Requirements
- Valid CA Driver's License, auto insurance, and clean driving record.
- Fingerprint clearance (DOJ/FBI).
Languages
Bilingual in English and Spanish preferred.

Physical Demands
Primarily sitting (80%), with occasional walking, standing, reaching, and lifting up to 20 pounds. Frequent speaking and listening. Visual acuity for various visual tasks.
