Summary
SUMMARY: Responsible for acting as a liaison between community mental health agencies and the Alameda County Medical Center outpatient psychiatric services program. Responsible for conducting initial screening of individuals for clinical appropriateness for treatment. Develops program relationships with community providers to enhances community education, integrate the program with the local mental health constituencies and improve the continuum of client care.

MINIMUM QUALIFICATIONS: 1. Education: Bachelor's degree or RN License with a minimum of 4 years experience; 2. Experience: 2 years working with seriously ill adults and 2 years working in healthcare related community education or outreach capacity; or a Masters degree in mental health, social work or human services field.
PREFERRED: Masters degree in Social Work, Counseling, Psychology or related field.
