Title: Intake Coordinator

Reports To: Administrator/Director of Patient Care Services
Summary: Responsible for implementing all aspects of the patient intake process including assessing patient needs and qualification, establishing and maintaining positive relationships with customers and referral sources, and managing the insurance verification and authorization processes. Acts as point of contact with referrer until patient is assigned to a staff member and coordinates clinical staff schedules.
Essential Functions:
Receives, evaluates and coordinates all incoming referral calls, including review of facility documentation, to assess that patient's needs can be safely addressed in the home, and that the patient qualifies for the service.
Gathers demographic and clinical information and verifies insurance benefit information including eligibility and service authorization procedures. Maintains current knowledge of insurance companies and change in practices.
Meets with supervisors to review all new referrals and confirm preliminary admission decisions.
Plans and coordinates assignment of clinical staff to clients. Acts as point of contact with patient and patient's family through the assignment of a clinical case manager.
Reviews scheduled staff to ensure skill level is adequate to meet patient needs.
Assists the Case Managers in the coordination of services with primary care providers, other health care providers, community partners, patients and their families.
Acts as a scheduler to ensure authorized visits are being completed in compliance with each patient's care plan and the qualifications and availability of each clinician.
For hospice referrals, explains services to patient and family members.
Promotes and maintains positive and professional relationships with discharge planners, case managers, physicians, physician office staff, and current and potential referral sources.
Gathers, collates, and reports referral statistics including key customer referral trends as requested.
Attends department meetings, as well as quality assurance and performance improvement activities, and contributes to the Quality Management review and evaluation.
Completes all required documentation in a timely fashion in accordance with regulations and office standards.
Maintains up-to-date clinical knowledge and enhances knowledge of home health and hospice guidelines.
Maintains comprehensive working knowledge of Self-Help HomeCare & Hospice's contractual relationships and ensures that patients are admitted according to contract provisions.
Ensures SHE and SHHCH policies are followed in the performance of duties.
Performs other duties as assigned.
Qualifications:
Current California Licensed Vocational Nurse.
Minimum one-year professional nursing experience in home health, hospice, acute or sub-acute preferred.
Preferred experience in healthcare operation and management.
Demonstrated ability in leading program intake effort.
Ability to engage and work collaboratively with others, including patients, patients' families, clinical team members, and community resources/partners.
Good communication, negotiation, and interpersonal skills.
Bilingual in English and Cantonese preferred.
Must pass health screening for TB clearance, physical examination, and background check.
Current CPR certification.
Must be able to travel via public transportation or personal vehicle for performance of job duties. If driving, must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage as specified in Self-Help's policy.
