Position: Program Coordinator

Department: Social Services

FLSA Status: Non-Exempt

Reports To: Director of Social Services

Job Summary: Responsible for coordinating and assisting with the operation of the Senior Escort Services.

Essential Functions:
Provide intake, assessment, scheduling, referral, and case management to clients who need escort services.
Supervise senior escort workers and assist the director with regular and ad-hoc reporting requirements.
Work with the program supervisor to build and complete an efficient program operations system.
Work closely with the Program Assistant and Volunteer Coordinator to recruit and train senior escort workers.
Schedule program work, oversee daily operations, coordinate the program's activities, and set priorities for managing the program.
Manage marketing and outreach to the community.
Respond to the concern and issues of clients and the senior escort workers.
Maintain an effective referral system with relevant city departments, organizations, and medical facilities.
Develop client database, senior escort worker, and driver scheduling system.
Maintain a resource database for the Senior Escort Services.
Attend meetings as assigned.
Prepares reports as required by funding sources.
Performs other duties as assigned.

Qualifications:
Bachelor's degree in social work, health, counseling, psychology, or related field with relevant social services experience.
Experience in providing health, social or aging services in the community.
Excellent verbal and written communication and presentation skills.
Must be self-motivated, organized, and able to multitask under minimal supervision.
Proficient in MS Office and the internet.
Bilingual in English and Cantonese preferred. Biliterate in Chinese is a plus.
Must be able to travel via public transportation or personal vehicle for performance of job duties. If driving, must have and maintain a valid CA driver's license, satisfactory driving record, and auto insurance coverage as specified in Self-Help's policy.
